First Free Class !

YES! I want to reserve my spot for a FREE class so I can try-before-I-buy. I understand in order to register and hold my spot in the free class (space is limited), I must COMPLETELY fill out and submit the contact information form. I acknowledge that introductory classes are held on Monday and Wednesday evenings at 7pm and will be there by 6:45pm to get started. 

I understand I am expected to show up for my scheduled free class on time and provide advanced notice if I am unable to make it. I understand if I do not show and offer no notice, I will be considered a "no show" and a $10 reservation fee will be required for future Free Class appointments. I understand my $10 deposit will be applied towards my full enrollment fee should I decide to join, or refunded to me after the free class if I decide not to join. I understand accountability is a critical element to achieving success with martial arts, self defense and fitness related training. Finally, I understand a United Source staff member will immediately contact me with confirmation and the details I need to know prior to my FREE class, or I can call your academy directly at 425-823-4415.
